This roles serves clients and requires travel throughout in Lake County, Porter County, LaPorte County, and surrounding areas.
As a Case Manager You Will Provide home based services to families involved in the child welfare system Conduct frequent home visits that will include needs and safety assessments of the parents and children in the home Coordinate community resources for the family Develop rapport and use a strengths-based approach with the families and children Position Requirements:
